- Medical history
- Social history
- Healthcare providers / caregivers
- Assessments
- Reports
- Patient Notes
How does NorthStar Dementia collect information about you?
We collect most of our personal information when you (or your guardian):
- Schedule an appointment / consultation with NorthStar Dementia
- Undergo consultations, assessments and / or therapies
- Contact us via our website
- Complete and submit a new client intake form
- Consent to have tests done as recommended
- Use or view our website via your browser’s cookies
- NorthStar Dementia may also receive your data from the following sources:
- Your doctors, healthcare providers or general physician
- Other doctors or specialists you have visited for treatment
- Caregivers and family members
- Health insurance providers
- NorthStar Dementia collects your data so that we can:
- Provide you with comprehensive dementia care and treatment
- Communicate with you regarding appointments, treatment, therapies, etc.
- Make claims to health insurance providers
- Obtain payment for services provided
- Law Enforcement/Legal Process
We may share your personal information with other people or businesses where we are required to do so by law.
- How do we store your information?
- NorthStar Dementia securely stores your information at our premises located at Russell Eve Building, Suite 205, 21 Church Street, Hamilton, Bermuda, HM11, where our building is secured with access mechanisms.
- Personal data is also stored digitally in our secure, password protected database and password protected files.
- We manage access to these files.
- We retain data for 7 years after a patient has died or left the practice, which is required by law.
- Third Party Vendors & Service Providers
NorthStar Dementia uses third party vendors and service providers to provide support and assistance for the treatment options we provide. We may disclose your Information to these service providers to the extent necessary to support our treatment programmes, therapies, operations, and services.
If we do this, we require these third parties to implement security safeguards to protect your personal information and to treat it in accordance with PIPA.
We do not allow our third-party service providers to use your personal information for their own purposes. We only permit them to process your personal information:
- for specified purposes, and
- in accordance with data privacy legislation.

NorthStar Dementia would like to make sure you are fully aware of all your data privacy rights.
Every client is entitled to the following:
- The right to access – you have the right to ask NorthStar Dementia for copies of your personal information. This right always applies. We may charge a small fee for this service.
- The right to rectification – you have the right to ask NorthStar Dementia to correct any information you think is inaccurate. You also have the right to ask us to complete information you believe is incomplete.
- The right to restrict processing – you have the right to request that NorthStar Dementia restrict the processing of your personal information in certain circumstances.
- The right to object to processing – you have the right to object to NorthStar Dementia’s processing of your personal data, under certain conditions.
